Health plans, hospitals partner to improve patient care coordination

Jacksonville, FLA., May. 19, 2015 – Availity, the nation’s largest real-time health information network, announced today the expansion of a new digital solution for hospitals in Oregon, Washington, Idaho, and Utah. The connectivity established by Availity creates a vital link for Regence physicians to electronically share members’ clinical information with the health plan’s care management team in the standardized HL7 data format. This secure, realtime information exchange platform enables providers and health plans to identify high-risk member populations and proactively coordinate care prior to a member’s discharge, thus closing care gaps and reducing the risk of readmission.

Availity’s solution also eliminates manual work and reduces coding gaps. By automatically extracting the electronic census data out of the hospital’s clinical management system and transmitting that to the health plan, Cambia Health Solution’s physicians can rely on the Availity platform to send notices to the health plan, eliminating manual phoning or faxing processes, and reducing the risk of missed notifications, claim denials, and uncompensated care.

In 2014, Availity partnered with Regence to establish connectivity with 17 hospitals in Oregon and Washington, replacing their manual clinical data intake processes with Availity’s platform. A positive outcome for Regence and their network hospitals resulted in the desire to expand the program, which now includes 100 hospitals across Regence’s four-state footprint.
